PREPARE FOR YOUR CHILD’S FIRST VISIT!
If you are scheduled for a first time visit, please make sure you bring immunization records to better assist you and your child. We cannot provide you with school forms at the time of your visit, unless you provide these records. Also please bring any medical history files if transferring care from another pediatrician.
Please prepare the following documents and bring them with you to your appointment.
Fill out our new patient forms before your appointment to save time!
- Insurance cards
- Complete immunization records
- Medical records from your previous pediatrician
- Letter of medical consent if someone other than a parent is bringing a child in
- New patient information
